Downloading HD Versions on YouTube

Is it possible to download the HD version (on YouTube for example) :?: . I always download in poorer resolution.

Yes, just select the output from settings of that video. Usually you need to restart video after selecting HD for Downloader to detect new stream.

However I'm quite new to using VSO Downloader, how do you proceed to adjust these settings?, and will the resolution be in real HD?

You don't change anything on Downloader, you click on that video window (it has "options") and change resolution from there. Then rewind or restart that video.
Sometimes it is quite tricky to get Youtube to send HD, but if it is available you usually get it after few tries.
And no, it will not be FullHD in most cases, it will be what ever Youtube is sending as you can not improve resolution of incoming stream. You can convert it to FullHD later, but that usually lowers quality.
