VSO CRASH

hello, can someone help me with a problem of VSO crashing and force closing after only a couple minute of use? This happens every time, not sometimes. and is very agrivating. i have sent a tech support report and they say the same thing. " You are not using recent version"
But i am. im using 4.0.0.19 any ideas? i have removed it and re installed several time. im running windows vista.thanks..CRASHDAWG

i unistalled vso, removed all folders. reinstalled and still force close. log file is empty?
