Subtitles not correctly processed when converting to MP4

I'm converting my Disney and Pixar collection from DVD VIDEO_TS folders to a MP4 format which will play on my iPads. I use the standard iPad profile for this.

About 50% of the movies aren't converted correctly on the subtitles front. The subtitles are being encoded in the correct language however a little into the movie the subtitles won't disappear to make way for the next one. For instance in Snow White the evil witch finished a sentence with "then she'll die". This text stays on the screen for the rest of the movie. New subtitles are placed on top of this and are barely readable.

I've tried to encode the movie three times, twice all by itself, but the results are the same every time, also on exactly the same moment in the movie.

Please advice.

Can you list a few more movies you might have had the problem with so that maybe we will have the same one in our office to reproduce the problem.
