How do I get the old default 'black mirror' menu back?

I just got the newest version and I previously used version 4.
I used the default 'black mirror' menu template and I liked it.
I don't see it listed in the menu options in this new version (current version as of 3 days ago).
Is there a way to get it back?

Hello again - I just read another post that claire replied to that shows how to get 'black mirror' menu back.
